To initiate help with a claim, you can contact us via email CSR@probityins.com or phone 408-293-9923 and identify yourself as an employee of your particular company.
To initiate the process, they will request that you complete a HIPAA release form enabling them to discuss your claim with the providers and insurance companies.
You will be notified of any progress if the claim requires scheduled follow-up. Once the claim is resolved, you will also be notified with information regarding the final resolution.
